Appointments
- You may now schedule appointments through January 2025. February 2025 appointments will be available by 2pm PT on November 20th.
- If you have a Zoom appointment scheduled, the link for your appointment is in your Secure Messages. (Note that each care team member has their own unique link which will be the same for every appointment with them.)
- Our clinicians occasionally run behind with their appointments. If the delay is longer than 15 minutes, a CCFM staff member will call to let you know. The same amount of time, attention and care will be devoted to your appointment. Please keep this in mind when scheduling your appointment as well as on the day of your appointment. Thank you for your understanding.
Set your password
This form lets you set a password for your patient portal account so you can access your medical information and send requests to your provider. For security purposes, we also ask you to provide some identifying information.
