Announcements

• Please note that the nutrition and health coach calendar will be temporarily closed for the month of September.  During this time, your clinician will be available to assist you with any questions, concerns, or immediate needs related to nutrition or health coaching during your clinician appointments or through secure messaging.  The Peer Support groups will also be paused until October.

Appointments

• You may now schedule appointments through November 2024. December appointments will be available by 2pm PT on September 20th.
• If you have a Zoom appointment scheduled, the link for your appointment is in your Secure Messages. (Note that each care team member has their own unique link which will be the same for every appointment with them.)
• Our clinicians occasionally run behind with their appointments. If the delay is longer than 15 minutes, a CCFM staff member will call to let you know. The same amount of time, attention and care will be devoted to your appointment. Please keep this in mind when scheduling your appointment as well as on the day of your appointment. Thank you for your understanding.
